2024-2025 Immunization Requirements

All students entering or attending school in NYS must be age-appropriately immunized.

·         Please refer to the 2024-2025 School Year New York State Immunization Requirements for School Entrance/Attendance form linked here for all vaccine requirements.

·         Also available on the NYSDOH Immunization website in the following languages: Spanish, Arabic, Bengali, Chinese, French, Haitian, Italian, Korean, Polish, Russian, Urdu, and Yiddish

Immunizations are due on the first day of school or students face school exclusion for noncompliance.  Kindergarten students must have all required immunizations even if they have not yet turned five years old.

If you do not have a health care provider or need help getting the immunizations, please visit the website for the Monroe County Immunization Program
